How to Register – New Students

New Students to DSS

  1. Log in to the Student Accommodation Manager (SAM)  and complete the New Student Application Form.  The New Student Application Form enables students to detail their disability, past services, and needed accommodations from their own perspective, and is helpful as an introduction to the Accommodation Coordinators they will eventually meet with.
  2. Submit documentation about your disability and/or past use of accommodations that will assist DSS in considering your requests by one of the following methods: upload in SAM, fax, email to dssoffice@udel.edu, or U.S. Mail.  Typically, documentation is from an appropriate, licensed professional who is able to detail the presence of the disability, the impact of it on the student’s learning or other major life activity, and relevant details to support the student’s request for accommodations. Some examples include medical records/letter from a physician or psychologist, psychoeducational assessment/evaluation, and school records. For the convenience of practitioners, certification forms have also been developed that may be completed for ADHD/ADD, Medical or Physical Disabilities, or Psychological Disabilities and are available for download at the bottom of this page. All documentation received is confidential and is kept in a private file at Disability Support Services.
  3. Schedule a Registration Meeting with a DSS Accommodation Coordinator.  During this meeting, the student and the Accommodation Coordinator discuss information that has been submitted and accommodations that may be appropriate. It is helpful that all information listed above has been submitted to DSS prior to this meeting, so that accommodations can be determined at that time. However if this is not immediately possible, please notify DSS and still schedule the registration meeting to get the process going. After this meeting and information is reviewed, DSS will notify the student of registration status via student’s UDel email address
  4. Students will use SAM each semester to request accommodation needs that need to be communicated or facilitated.  Please refer to the UTILIZING ACCOMMODATIONS page for instructions. Students must log into SAM to request that Faculty Notification emails be sent to their instructors.  For classroom accommodations, the instructors are typically notified via email within 48 hours of when students log-in to make their request. In some cases, the DSS office may make direct arrangements on behalf of the student rather than email (i.e., ASL interpreters, housing accommodations).

IMPORTANT TO NOTE: Once accommodation emails are sent out, students are required to follow up with each instructor to introduce themselves and to discuss how the accommodations will take place in each course.

If it is more convenient than accessing prior records, students may have their healthcare provider complete the appropriate form from those listed below.
