Background Clearances for Internships/Practicums

If you are registered for an Internship or Practicum you may be required to complete a background check and other additional clearances depending on your internship/practicum site.  If you are unsure if your internship requires clearances, please check with the instructing professor.

Please note that clearance paperwork needs to be turned in and approved before you can begin your rotation.  Not having the appropriate clearance paperwork in place by the deadlines below could result in your internship being delayed or being dropped from the class.

BHAN263 - Leadership Practicum - Christiana Care Rotations

To begin your internship you will need to turn in the following paperwork:

  • Background Check (through CastleBranch)
  • Drug Test (through CastleBranch)
  • PPD Test
  • Vaccination Records
  • Flu Shot (Nov-Mar)

Please turn in all paperwork and results to Carrie Johns before the first day of classes. Not having these required items turned in will cause a delay in your internship.

  1. Negative PPD test needs to be valid within the last year (rolling 365 days).
    • Test can be provided by Student Health ($10.00 fee**).
    • Results from a Primary Care Physician (PCP) must be turned in Carrie Johns. Results can be faxed to 302-831-4261 or e-mailed to cbonnett@udel.edu.
    • Please note: PPD Tests require two visits to physician/student health 48-72 hours apart. Be sure to plan accordingly and do not wait until the last day to schedule your PPD test.
  2. Flu Shot needs to be for 2019-2020 flu season.
  3. Vaccination Records: there should be a copy of these in your Student Health Profile. You can take a screen shot (showing your name and UDID) and e-mail it to Carrie Johns at Cbonnett@udel.edu, or you can have your PCP fax them to Carrie Johns’ attention at 302-831-4261.
    • The required vaccinations are:
      • Measles, Mumps, Rubella (MMR) or positive titer
      • Varicella or positive titer
      • Hepatitis B or waiver

  1. Go online to portal.castlebranch.com/ue74
  2. Select Place Order
  3. You will be given three package options – select “UE74: Background Check – Drug Test” ($70.00 Fee**)
  4. Follow the instructions to get set up and create a username and password
    1. Note – Not having a username will result in your background check and drug test results never being reported back.
  5. Contact Carrie Johns to schedule a time to pick up a “Chain of Custody Drug Form” for your Drug Test
  6. Go to Lab Corp to complete drug test. You do not need to schedule an appointment and any location is sufficient.
  7. View and download your Background Check/Drug Test results in your CastleBranch profile
  8. Turn in result to Carrie Johns

Only once all items are turned in, Dr. Bibik/Dr.Lodyga will review your paperwork and approve your rotation start date.

It is very important to read these directions carefully.

All documentation must be turned into Carrie Johns (contact information below) no later than Friday, December 20, 2019. Not turning in documentation or turning in documentation late WILL result in a delay in access to the facility for not just you, but the whole class.

REQUIRED DOCUMENTATION:

  1. Personal Health Insurance Card (copy of front and back)
  2. Background Check
  3. Drug Screen
  4. Immunization Records
  5. PPD Test
  6. Flu Shot
  7. Clear Screen Shot of Completed Policy & Procedure Quiz (minimum score of 85%)
  8. Clear Screen Shot of Completed Pre-Orientation Quiz (minimum score of 100%)
  9. Signed Confidentiality Agreement
  10. Signed HIPPA-106 Security Oversight
  11. Signed Orientation Acknowledgement

BACKGROUND CHECK/DRUG SCREEN:

  2. Select Place Order

*If you have had a Criminal Background Check completed within the last rolling year, you can use this and select the Drug Screen only option.

    • Test can be provided by Student Health ($10.00 fee**) in needed
  2. Flu Shot for 2019-2020 flu season.
  3. Vaccination Records including
    • Measles, Mumps, Rubella (MMR) or positive titer
    • Varicella or positive titer
    • Hepatitis B (3 shot series) or waiver

These results can be scanned and e-mailed, faxed, or delivered in person to Carrie Johns.

**Students are responsible for all fees associated with clearances

HEALTH INSURANCE CARD

  1. Photo of front and back of card sent via e-mail
  2. Carrie Johns will photo copy your card if you bring it to her office

ONLINE QUIZZES/SIGNED DOCUMENTATION

All of the information for orientation, online quizzes, and signed agreements/acknowledgments are in the PDF File from Union Hospital. Please read these directions carefully and complete all the necessary steps.
